Who do you support?
We support children, teenagers and adults with disability aged 6 to 65, across Western Sydney (Blacktown) and the NSW Central Coast.
Are you a registered NDIS provider?
Yes. Passion Flower Disability Pty Ltd is a registered NDIS provider (registration number 4050128580), regulated by the NDIS Quality and Safeguards Commission.
How is support funded?
Our supports are funded through your NDIS plan and charged in line with the NDIS Pricing Arrangements and Price Limits. We will go through your plan with you before any support starts.
How do I get started?
Call us on 0448 402 108 or 02 8814 8714, email info@passionflowercare.com.au, or complete our online referral form. We will meet with you to understand your goals and plan your supports.
Can support coordinators make a referral?
Yes. Support coordinators, plan managers and families can all refer through our online referral form or by calling us.
Do you support people with complex or high care needs?
Yes, this is our specialty. Our trained support workers provide high intensity personal care, including complex bowel care, catheter and continence management, PEG feeding, seizure management and dysphagia support, in line with each person's health care plan.
Do you have Supported Independent Living (SIL) vacancies?
We currently have vacancies in two SIL homes, one in Blacktown and one on the Central Coast, both with 24/7 support. See our SIL Vacancies page or call us to arrange a visit.
Do you need an interpreter?
Call the Translating and Interpreting Service (TIS National) on 131 450 and ask them to call Passion Flower Care on 02 8814 8714. If you are Deaf or have a hearing or speech impairment, you can contact us through the National Relay Service.

I accessed Passion Flower Care for one of my NDIS participants who was lacking in motivation due to his limited access to the community. Passion Flower Care came onboard with the participant and spent a lot of time getting to know the participant and engaging him in activities of his choosing in the community. The best thing about Team Passion Flower care is they actually listen to you and your requirements. They are proactive and plan out their sessions before coming to pick the client.
